Jordan Valley Health Center will be on-site at Republic High School on April 15 from 8:00 a.m. to 2:00 p.m.and at Republic Middle School on April from 8:00 a.m. to 2:00 p.m. to provide school-required vaccines to eligible students who would like them. Sports physicals will also be available on these dates. To learn more and to sign up your student, contact your school nurse (RMS or RHS).
Immunizations will be billed to the student's insurance plan if they have coverage. Those who have Medicaid, no insurance, or insurance that does not cover vaccinations will be covered by the Vaccines for Children program. Sports physicals are considered self-pay and will be $10.00 at the time of the visit. This can be paid by cash, card or check.
